When it comes to understanding your passport card form, you will find that it is pretty much a straightforward form easily fillable if you know what you are doing. As a first time applicant for the US Passport card, you will be required to submit Form DS-11. You must submit Form DS-11 if any of the following apply to you:
-- You have never been issued a U.S. passport or -- You are under age 16 or -- You were under age 16 when your previous passport was issued or -- Your most recent U.S. passport was issued more than 15 years ago or -- Your most recent U.S. passport was lost or stolen or -- Your name has changed since your previous U.S. passport was issued and you are unable to legally document your name change
First, you will need to complete the form by hand. Write legibly and only use black ink. Illegible application, application in anything but black ink, and incorrect information may cause your application to be denied or delayed. One important thing to consider is that the US Passport card is not globally interoperable and, therefore, is not applicable for travel via air either to or from the United States. If you plan on traveling by air or an emergency arises where air travel is a necessity, the US Passport card is NOT for you! The US Passport card is only valid for travel by air or sea to or from Bermuda, the Caribbean, Mexico, and Canada.
Form DS-11 is actually only 2 pages. However, you must understand and fill out each form field that applies to you and your situation.
